In order to receive an exchange the following criteria applies:

1.  The unit must be in the original box with all the packing and 
    accessories.

2.  The serial number on the unit has to match the serial number on the
    box.

3.  No writing or marking on the box.

4.  Less than 30 minutes talk time on the phone.

If the phone does not meet all these criteria, it will be repaired and
returned to your store.

Steps required in  returning a telephone:

1.  Fill out all the information on the form below.

2.  Fax it to Darlene Daniel at National Auto Center @ 214-257-1708.

3.  She will assign an authorization number for the return and fax the
    form back to you.

4.  Ship the merchandise to the address below listing the authorization
    number on the outside of the shipping carton.  Include a copy of the
    completed form with your return.
